This was my first ever marathon and I'm glad I did one that was both flat and small. You start the race at Kitty Hawk, then run through Kill Devil Hills, Nags Head, then cross the bridge to Manteo. The beginning will be crowded, but by mile 3 or 4 everything loosens up. Plenty of portopotties at the beginning as well and they have a bag drop off (didn't use it but they have a UPS truck there).

If you are looking for a ton of crowd support this probably isn't the race for you. There are spectators here and there but there will be times that it is just you and the pavement. One highlight is that you will get to run around the Wright Brother Memorial which is pretty cool. Water stops are well stocked, but nothing fancy like you will find at bigger races.

The course is really flat (it being at the coast and all), but beware of the bridge as you cross from Nags Head to Manteo. This is a pretty good climb considering all you have been running is flat terrain the past 20ish miles, so make sure you have some fuel in the tank to get over it.

The year I did it I was a little disappointed in the medal (fairly small for a marathon medal). Hopefully they have gotten a little bigger since then.

Overall a great race to do in NC to check off your 50 state list (especially if you want a flat one in NC)!
